Direct Support Professional - Day (3072)

RESPONSIBILITIES
Assists with the assessment, development, and implementation of individual service plans, provides quality care, training, support, and supervision, assists individuals with self-help, task learning, and community utilization skills based on individual goals; maintains individuals daily records and documentation according to all applicable standards; assists with planning and implements group day, community engagement, and community coaching services to meet individual needs; monitors and maintains the safety and wellness of individuals; communicates with other community agencies and programs; and provides transportation to implement the individuals Individual Service Plan.Hours of the program may include weekdays, weekends, and evenings.

REQUIREMENTS
Minimum Requirements
M1: High school diploma or equivalent
M2: Valid drivers license and safe driving record

Fingerprints, State and FBI criminal record reports, drug test, and central registry (CANIS) report will be required upon request. Our Agency maintains a drug-free workplace.

ANNUAL SALARY RANGE
$31,866 - $55,748
